H&P #1

Ideally a complete H&P

Only the History and Physical Exam findings need to be written out this time.

 

·     If needed, initial meeting with student to meet you and your staff and for orientation activities (IDs, parking, special instructions, etc.)

·     Student conducts FIRST complete H&P on real patient. Student emails H&P to preceptor and Jacqueline Long. (No identifying information).

·     Preceptor reviews and critiques H&P (use Write-up Evaluation Form – Sent via e-mail. Review with student and send evaluation by email to student and Jacqueline Long.)

October 26

H&P #1 AND Evaluation is due.

 

H&P #2

Ideally a complete H&P.  Written requirements: H&P; List of pertinent positives and negatives; Assessment and 3-part Plan; Problem list; Admitting orders (if applicable)

 

·     Student conducts SECOND complete H&P.

·     Preceptor reviews and critiques H&P (use Write-up Evaluation Form – Sent via e-mail, Review with student and send copy of evaluation by email to Jacqueline Long.)

December 14      

H&P #2 AND Evaluation is due.

Holiday Break

HOLIDAY BREAK: not required to meet during this time period.

 

H&P #3

Ideally a complete H&P.  Written requirements: H&P; List of pertinent positives and negatives; Assessment and 3-part Plan; Problem list; Admitting orders (if applicable)

 

·     Student conducts THIRD complete H&P.

·     Meet with student to review and critique THIRD H&P (use Write-up Evaluation Form – Sent via e-mail, Review with student and send copy of evaluation by email to Jacqueline Long.)

February 8       

H&P #3 AND Evaluation is due.

 

H&P #4

May do focused H&P; List of pertinent positives and negatives; Assessment and 3-part Plan;  Problem list; Admitting orders (if the patient is admitted)

 

·     Student conducts FOURTH  H&P, may be focused H&P

·     Meet with student to review and critique FOURTH H&P (use Write-up Evaluation Form – Sent via e-mail, Review with student and send copy of evaluation by email to Jacqueline Long.)

Spring Break

SPRING BREAK: not required to meet during this time period.

March 22

H&P #4 AND Evaluation is due.

 

H&P #5

May do focused H&P; List of pertinent positives and negatives; Assessment and 3-part Plan;  Problem list; Admitting orders (if the patient is admitted)

 

·     Student conducts FIFTH H&P, may be focused H&P

·     Meet with student to review and critique FIFTH H&P (use Write-up Evaluation Form – Sent via e-mail, Review with student and send copy of evaluation by email to Jacqueline Long.) 

·     Preceptor completes Student Performance Evaluation Form and provides feedback to student and Jacqueline Long via e-mail.

April 18

H&P #5 AND Evaluation is due.
