Dear Medical Student:

The Loyola Occupational Health Service welcomes you to the Loyola University Health System and the Stritch School of Medicine!

In order to ensure your compliance with occupational health and safety recommendations, prior to starting school, you will need to provide documentation for your student health record. Forms used at Loyola may be very similar to the documents you have on file at your undergraduate school. Also, immunization information is likely most easily available through the student health record.

So that you can start your classes without delay, please return the following information by July 5, 2009 to:

  1. Loyola Evaluation History Form – Please complete. The information on this form will be used to generate an employee health record number and serve as the basis of your confidential student health record.
  2. OSHA Respirator Medical Evaluation Questionnaire – Please complete. Answers to these nine questions will be used with your N95 fit testing to qualify your respirator use at Loyola.
  3. Immunization records – Please request a copy of your immunization records from your school health office and send with your completed forms. To begin classes, your health record must include proof of immunization or protective titers as follows:
    1. Measles (protective titer or evidence of two immunizations after 12 months of age)
    2. Mumps (protective titer or evidence of two immunizations after 12 months of age)
    3. Rubella (protective titer or evidence of one immunization after 12 months of age)
    4. Varicella (protective titer, documentation of two doses of vaccine, or history of illness)
    5. Hepatitis B immunity (evidence of immunization and/or titer)
    6. Report of TB skin test applied and read during 90 days prior to assuming clinical responsibilities (or chest x-ray report done during 90 days prior to assuming clinical responsibilities).

The form "Occupational/Student Health Services Infection Control Screenings" is used to document the results you provide
